Nobody likes filling out paperwork...unfortunately it NEEDS to be done! We have tried to make it as easy as possible!
We REQUIRE a parental consent and waiver to be filled out for EACH youth. The good news is: it is good for the entire school year! The only other time you will need to fill out a form is when we travel internationally or if we partner with another organization that requires it.
